Completely Care Ltd are recruiting on behalf of our client for a new role focused on transitioning young people from supported accommodation into independent living.
The selected candidate will work closely with providers of supported housing to identify individuals who are prepared for and interested in progressing to independent living arrangements.
This role prioritises single homeless adults with minimal support needs who are ready for transition to independence or other more fitting living situations.
A significant part of this role involves circulating strategies across the network of accommodation services, which assists staff to facilitate successful transitions for their clients.
Core Responsibilities:
Collaborate with supported housing services to identify clients ready for and interested in moving to independence.
Locate appropriate housing options, liaising with local authority schemes, and removing obstacles to access for participants.
Assist individuals in exploring housing options and overcoming personal challenges to secure sustainable living situations.
Develop personalised support plans in collaboration with current housing providers, reflecting the client’s desires, needs, and goals within available housing options.
Partner with the Access Panel’s administrators to prioritise efforts on services with extensive waiting lists, enhancing move-on efficiency.
Increase availability within supported housing by assisting those most in need, such as individuals experiencing homelessness.
Share best practices and learnings with partner organisations to improve skills and contribute to the evolution of services.
Participate in and contribute to initiatives aimed at enhancing the transition process from supported housing.
Support clients in building external relationships, fostering respect, dignity, and community integration.
Guide clients in developing skills and experiences beneficial for independent living, such as budget management, self-care, and life skills.
Uphold the organisations values and methodologies, emphasizing strength-based, client-centered, and trauma-informed approaches for meaningful and lasting recovery.
Conduct safety planning, risk assessments, and management plans in line with safeguarding policies, in collaboration with the safeguarding lead.
Facilitate client access to core programs for further opportunities in employment, education, and personal development.
Participate in professional development activities to maintain the effectiveness in the role.
Maintain accurate and up-to-date case notes and data for reporting purposes.
Represent the organisation in external engagements, including events, networks, and collaborative groups.
