Want to end youth homelessness through prevention? Young people who are transitioning from high level support, need more assistance when moving out of foster or residential care and out in to the community. Changing environments can be daunting...The Enhanced Transitional Support programme (ETS) is a brand new project that has been set up in partnership with Neath Port Talbot local authority, to help make the transition easier and more effective. You will provide specialist, enhanced housing and related transitional support to vulnerable young people, within 24 hour supported accommodation. Your role will be an important one; you'll be the person they can rely on for help, advice, guidance, emotional and practical support. (Please note, there will be NO personal care involved in this role, it is NOT a health care position). As the Support Worker, you will be based in the local area and support a small caseload of young people. You will work with the project based Support Assistants and cover the days (9.30am to 5pm). What will you do? Support vulnerable young people to reduce the likelihood of homelessnessAssisting young people to become more independent and self-sufficientProvide day-to-day emotional and practical support Be a guide and a mentor to those who need it most Keep young people safe from harm by reducing risk Achieve great outcomes by encouraging positive behaviours Work with other organisations to provide the best level of support and advice What do you need? Experience of working with vulnerable young people A background in Social Care, Support Work, Education or similar Good understanding of safeguarding proceduresStrong health, safety and well being knowledge gained in supportive environment An understanding of trauma, ACE's and mental health issuesKnowledge of housing and homelessness would be helpfulWilling and able to carry out a new DBS check and provide at least two employer referencesOwn transport is essential Other info...
