Registered General Nurse in Padgate

Search are currently looking for Band 5 Registered General Nurses to work within the NHS in the Warrington areaIdeally they would like someone full time however, adhoc shifts are available.

They are looking for both night and day staff.

You will enable people to be cared for by providing rehabilitation for both step down and step up patients you need to have a good all round skill set to support the needs of these patients.

Duties:Ensuring a high standard of care with intermediate care bed based facility.

Liaises with the Intermediate Care Sister / Charge Nurse to ensure information in relation to patient care/changes to care plan and the continuity of care.

Liaises with all health care professionals and other agencies to ensure effective service delivery in the intermediate care facility.

Perform comprehensive assessment of patient nursing needs.

Assess, plan, implement and evaluate programmes of care according to changing health needs under the direction of the Intermediate Care Sister / Charge Nurse.

Maintain accurate records in accordance with NMC.

Attend a multi-disciplinary case conference as the named nurse if appropriate.

To communicate and collaborate with other members of the Integrated Intermediate Care Team, PHCT, voluntary and statutory organisations to support individual patient care.

To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.

To competently perform clinical skills related to the post.

To be professionally responsible for delegated visits under the supervision of the Intermediate Care Sister / Charge NurseTo work without direct supervision.
