Care Assessor in Ferndown

Job Description

Are you an experienced care professional? Altogether Care is seeking a Care Assessor to join our team and work alongside our registered manager and community care team.

In this role, you'll have the opportunity to make a significant difference in people's lives. If you're organised, solution-focused, and calm under pressure, we want to hear from you!

As a Care Assessor with us, you'll be responsible for

• Carrying out assessments of all new packages

• Organising care plans, assessments, and reviews

• Conducting required risk assessments and audits to ensure safe care

• Attending reviews for new and existing clients

• Supporting the on-call service on a rota basis and undertake care work, if required

• Coordinating staff rotas

• Building relationships with care staff and clients

• Liaising with other health and social care professionals
